Pre Algebra Test Practice

Could someone give me a few practice Algebra Problems dealing with line slope and (h,k) form?
In Pre-Calculus, we’re doing an algebra review and a test is coming up this week. I understand the problems, but I feel I need to practice more. Help?
Right now, we’re dealing with any sort of slope-intercept equation problem (given two points, an equation and a line [find parallel or perpendicular], given the x- and y-intercept, etc.) and writing problems such as 2x^2+4x-9 in (h, k) format.
Like this: y=a(x-h)+k
1) Slope of a line passing through (3,4) and (6, -6)
2) Equation and slope of line passing through (0,5) and (5.-5)
